PARENTAL TRAVEL RELEASE FOR A MINOR

IF YOU ARE UNDER 18 YEARS OLD ON THE DATE OF YOUR
TRAVEL AND WILL NOT BE ACCOMPANIED BY BOTH LEGAL
GUARDIANS YOU MUST FILL OUT THIS FORM TO BE
PRESENTED AT AIRPORT CHECK-IN AND TO IMMIGRATION
OFFICIALS.

Foreign governments require travelers under 18 years of age to present a letter from their parents permitting them to travel outside the U.S. and Canada. Foreign immigration takes this matter very seriously. If ALL of the following are not done you will be denied entry into the country and returned to the U.S. at your own expense and thereby forfeit your entire
trip. This form must be filled out in the presence of a NOTARY PUBLIC. BOTH natural parents must sign, even if divorced. If divorced with sole custody, legal documentation supporting this must be attached and notarized. If natural parent is deceased, a certified copy of the death certificate must be attached. Step-parents can NOT sign, unless child has been legally adopted, in which case, legal documentation supporting this must be attached and notarized.
